Meet Dale, our dedicated Administrator at Carolina Rivers Nursing & Rehab, a seasoned professional with a passion for compassionate care and a wealth of experience. With over 30 years of marriage to his wonderful wife and three grown children, Dale understands the importance of family values. A proud member of the American Legion and Rotary, Dale’s commitment to community service is deeply ingrained. Dale’s journey in healthcare began in the U.S. Navy, where he served for nine years as a Hospital Corpsman, stationed with various Marine Corps units. During his tenure, he specialized in Physical Therapy, a field he continued to pursue for about 30 years. His dedication led him to Long Term Care assigned to various facilities within Principle LTC, where he spent 12 years, including Carolina Rivers, initially as the Rehab Director for 3 years.

Driven by a desire to do more for residents and staff, Dale transitioned his career a few years ago. He took the leap to become a licensed Nursing Home Administrator, a decision he considers his true calling in life. Dale’s leadership is grounded in his 30 years of experience, starting with his U.S. Navy service and developing his leadership skills from the ground up, including 5 years at Carolina Rivers as the Rehab Director and Administrator. He successfully completed Principle LTC’s Administrator-in-Training program, earning his license and leading our diverse team of professionals.

Kelly Porterfield

Director of  Nursing

Kelly has excelled as the Director of Nursing at Carolina Rivers for over four years, with a career of more than two decades dedicated to various roles within the long-term care community. Starting her nursing journey in West Virginia, Kelly later contributed over a decade as a Director of Nursing in Florida. Upon relocating to Charlottesville, VA, she continued her commitment to long-term care, taking on pivotal roles such as Director of Nursing, Travel Nurse Consultant, and Regional Nurse Consultant. Since 2018, Kelly and her husband, now empty nesters, have called the Jacksonville, NC area home, drawn by its coastal charm. Residing near the ocean, she finds fulfillment in both her professional and personal life in North Carolina. Leading the diverse clinical team at Carolina Rivers, Kelly directs her focus on enhancing outcomes and improving the quality of life for residents, while supporting families and keeping them informed.

Admissions Director

With over 20 years of experience in skilled nursing, Tracey King proudly serves as the Admissions Director at Carolina Rivers Nursing & Rehab. Having held various roles from unit clerk to central supply and receptionist, she brings a wealth of relevant experience to her position. Living just three minutes from Carolina Rivers, Tracey is hands-on with every admission, extending a warm welcome to families and encouraging them to share their questions and concerns. Tracey’s personal connection to skilled nursing began during her grandfather’s time in hospice, fueling her passion for ensuring a seamless experience for new residents. Drawing inspiration from the stories of the older generation, she values the unique history they share. As a mother of five and grandmother of five, family is at the core of her values. With a deep appreciation for local military stories, she loves Jacksonville’s close-knit community, where the unique histories of its residents enrich her understanding in a way that goes beyond the pages of any book.
